According to the legislative decree n. 196/2003 (Code concerning the protection of personal information), which has replaced the law n. 675/1996, the handling of information concerning you will be based on principles of fairness, legality, transparency, and protection of your privacy of your rights.
According to Article 13 of the legislative decree n. 196/2003 (formerly Art. 10 law 675/1996) of the above law, therefore, we provide the following information:
The information you provide will be used, within the limits of privacy legislation, for the following purposes: protection by all legitimate means, and in particular with recourse to legal instruments, the rights and interests of consumers and users. The Codacons believe that only citizens aware of their own rights can defend themselves in the breach of said rights; for this reason it is the primary objective of the Association to provide continual and updated information on the initiatives in defense of the following types of consumers: social, economic, financial, political and institutional, national and community.
The data will be handled using paper and/or computers.
The provision of data relating to name, last name, city, telephone number, and email is required in order to offer the service for information, advice, assistance or services you request and the refusal to provide such data could lead to failure or incomplete performance of the services provided; the provision of data relating to telephone and email address is optional and is designed to enable us to inform and update you on the activities of the clinic and the services provided. We inform you that the personal data that could reveal racial or ethnic origin, religious, philosophical or other beliefs, political opinions, party membership, unions; religious, philosophical, political or trade union associations or organizations, as well as personal data disclosing health and sex life, those relating to health, are sensitive information. This information will not be processed without prior express written consent;
The information will be communicated, subject to their consent, to other parties (organizations or associations) that share the intended purpose defined by the Statute of the Codacons in relation to the defense of the rights and interests of the consumers and users.
The title of the management is Ortho-Spine Srl, based in Monza (MB), Corso Milano 31, and its Director, Dr. Pedro Luis Berjano. The processing of information takes place at the above-mentioned office and will be handled only by the personnel responsible for it.
At any time you can exercise your rights towards the manager of the data, in accordance with art. 7 of the Privacy Code (formerly Art. 13 of Law n. 675/1996), in particular you can request to know of the existence of your personal information, to obtain without delay the communication in intelligible form of your personal information and their origin, the cancellation, anonymization or blocking of personal data that are in violation of law; bringing up to date, the rectification or integration of information; the certification that the above functions have been made known to those with whom the data were disclosed, except where this proves impossible or involves a manifestly disproportionate to the protected right; to object, in whole or in part, for legitimate reasons, the processing of your personal data, pertinent for collection purposes.
